BACKGROUND AND OBJECTIVE: Atypical nevus syndrome has been described as one of the main risk factors for melanoma. The aim of this study was to analyze dermoscopic changes observed in melanocytic lesions over a follow-up period of 5 years in patients with atypical nevus syndrome. MATERIAL AND METHODS: We conducted a retrospective follow-up study of a cohort of patients seen at a specialized skin cancer and digital body mapping clinic in Medellin, Colombia, between January 2017 and December 2022. We analyzed the dermoscopic changes observed during this period and explored their association with newly diagnosed melanoma. RESULTS: A total of 368 patients (187 women) with a median (interquartile range) age of 43 (37-51) years were included. The dermoscopic features observed at 5 years were an atypical network (222 patients, 60.3%), asymmetric globules (163, 44.2%), white-gray regression areas (105, 28.5%), lesion regression (72, 19.5%), a negative pigment network (59, 16%), asymmetric eccentric pigmentation (28, 7.6%), asymmetric projections (21, 5.7%), and asymmetric vascular patterns (8, 2.1%). Melanoma was diagnosed in 12.2% of patients during follow-up. Features significantly associated with a shorter time to melanoma onset were grayish-white areas (P<.001), asymmetric globules (P=.011), asymmetric eccentric pigmentation (P=.047), and a negative pigment network (P=.001). CONCLUSIONS: The main dermoscopic features of melanocytic lesions in patients with atypical nevus syndrome associated with progression to melanoma were grayish-white areas, asymmetric globules, asymmetric spots, and a negative pigment network.

Chronic trauma has been hypothesized to play a role in the development of this tumor. The aim of this study was to analyze the association between the presence and characteristics of ALM and location in stress- and non-stress-bearing areas. MATERIAL AND METHODS: Observational, descriptive analysis. For 95 patients with ALM evaluated at Fundación Clínica Vida in Medellin, Colombia between January 2016 and August 2020 we collected demographic, histologic and clinical variables and assigned the patients to 2 groups depending on whether their tumor was located in a stress-bearing area or not. RESULTS: Ninety-five patients (36 men and 59 women) with a median age of 59 years were studied. ALMs were more common in stress-bearing areas than in non-stress-bearing areas (58 [61%] vs. 17 [17.8%]). A history of direct trauma at the site of the tumor was reported in 18.9% of the cases (p = .025) and was most common in patients with nail lesions. Invasive ALMs in stress-bearing areas had a greater Breslow thickness (p = .006) and higher ulceration rates than those in non-stress-bearing areas (p = .018). CONCLUSIONS: ALM on both the feet and hands is more common in stress-bearing areas. Our findings support the hypothesis that mechanical stress may have a pathogenic role in ALM.

Antecedentes y objetivo: El melanoma lentiginoso acral (MLA) se presenta en zonas con poca exposición a la radiación ultravioleta. Se ha planteado que el trauma crónico puede predisponer a su desarrollo. El objetivo del presente estudio fue relacionar la presencia del MLA y sus características con las zonas acrales que se consideran de presión y de no presión. Material y métodos: Se realizó un estudio observacional, descriptivo, con intención analítica. Se recogieron los datos de 95 pacientes con diagnóstico de MLA evaluados en la Fundación Clínica Vida en Medellín, Colombia, entre enero de 2016 y agosto de 2020. Se recolectaron variables demográficas, histológicas y clínicas del tumor, las cuales se analizaron en dos grupos, según si estaba sometida a fuerzas de presión o no. Resultados: Se incluyeron 95 pacientes con MLA (36 hombres y 59 mujeres), con una mediana de edad de 59 años. Se presentaron más melanomas en las zonas de presión que de no presión (58 [61%] vs. 17 [17,8%]). Se refirió la existencia de un trauma directo previo en 18,9% de los casos, el cual fue más frecuente en las lesiones de las uñas (p = 0,025). Los melanomas invasores tuvieron un mayor Breslow (p = 0,006) y presencia de ulceración (p = 0,018) en las zonas de presión y en las uñas que en las zonas de no presión. Conclusiones: El MLA es más frecuente en zonas de presión que de no presión, tanto en la mano como en el pie. Esto apoya la hipótesis de que el estrés mecánico puede jugar un papel en la patogénesis de este tipo de tumor (AU)

INTRODUCTION: Glaucoma is a chronic asymptomatic disease, progressing to loss of vision. Elevated intraocular pressure is the only modifiable factor. Adherence to glaucoma treatment varies from 2-80%. OBJECTIVE: To evaluate factors associated with adherence to topical glaucoma treatment reported in the literature, and to identify protective factors and risk factors, as well as modifiable and non-modifiable factors, in order to take them into account to perform interventions in adherence. MATERIALS AND METHODS: A bibliographic search of articles published in the last 8 years in databases such as Clinical Key, Cochrane (OVID), EBSCO, Lilacs, PubMed and Science Direct, of different observational studies that performed a measurement of the treatment of glaucoma and to identify associated factors. RESULTS: A total of 7 studies were selected that directly and indirectly measured adherence to glaucoma treatment. Different risk and protective factors for adherence to treatment were found. These showed that African-American race, poor education, low personal income, and high treatment costs, are strongly related to poor adherence to treatment. It was also found that educational interventions and a good patient- physician relationship impacted positively on adherence. CONCLUSION: The results found are a guide for risk and protective factors for adherence to treatment of glaucoma. It is cost effective to educate patients to positively impact adherence. By identifying such factors, attention can be focused on poor adherence patients.

OBJECTIVE: To evaluate recent evidence concerning the relationship between the exposure to organic solvents and the impairment of colour vision. METHODS: A bibliographic search was conducted for scientific papers published in the last 15 years, in the LILACS, PubMed, Science Direct, EBSCO, and Cochrane databases that included observational studies assessing the relationship between impairment in colour vision and exposure to organic solvents. RESULTS: Eleven studies were selected that were performed on an economically active population and used the Lanthony D-15 desaturated test (D-15d), measured the exposure to organic solvents, and included unexposed controls. It was found that there is a statistically significant relationship between the exposure to organic solvents and the presence of an impairment in colour vision. CONCLUSIONS: The results support the hypothesis that exposure to organic solvents could induce acquired dyschromatopsia. The evaluation of colour vision with the D-15d test is simple and sensitive for diagnosis. More studies need to be conducted on this subject in order to better understand the relationship between impaired colour vision and more severe side effects caused by this exposure.
